Service Description
For established patients: a 30-minute follow-up via secure telehealth to review your progress, discuss your response to treatment, adjust medications when appropriate, and review your ongoing care plan. Cash/self-pay rate: $150 for 30 minutes. The price shown is the cash/self-pay rate. If you plan to use insurance, enter your insurance details on the intake form so we can verify your coverage before your session. Your copay, deductible, or other patient responsibility depends on your verified benefits. Booking an appointment does not confirm insurance coverage. Required before you can be seen Complete the intake and consent forms before your appointment. If your forms are already on file, keep your insurance information current and complete any updates requested by the practice. Cancellation Policy
